It will probably be this week&#8217;s least-favorite task. The form&#8217;s nine pages let me outline how I see medical decisions. And who I&#8217;d like to make those decisions for me if I can&#8217;t.<\/p>\n<p>Picking my primary and secondary health care agents was the easy part, thanks to my being part of a close-knit family.<\/p>\n<p>Making choices about organ donation, autopsy and what to do with my body after I&#8217;m dead wasn&#8217;t difficult.<sup><a href=\"https:\/\/brendans-island.com\/catholic-citizen\/health-care-directive\/#1\">1<\/a><\/sup> But it didn&#8217;t brighten my day.<\/p>\n<hr>\n<h3><a name=\"changing\"><\/a>Changing Times: Coffins, Cholera and Care for the Sick<\/h3>\n<h4><a name=\"options\"><\/a>Options of Yesteryear<\/h4>\n<p><a href=\"https:\/\/brendans-island.com\/catholic-citizen\/different-sorts-of-dead\/#presumed\"><img data-recalc-dims=\"1\" decoding=\"async\" src=\"https:\/\/i0.wp.com\/brendans-island.com\/blogsource\/20160321ff\/800px-Premature_Burial_Vault-658.JPG?w=640&#038;ssl=1\"><\/a><\/p>\n<p>My options would have been different in the 19th century. Different, but still unsettling.<\/p>\n<p>The era&#8217;s health care tech and assorted epidemics encouraged skittishness when examining possibly-contagious corpses.<\/p>\n<p>Waking up in a coffin was a real possibility.<\/p>\n<p>My guess is that odds for waking up in a body bag are lower these days.<\/p>\n<p>I&#8217;ve got mixed feelings about death.<\/p>\n<p>Ideally, &#8220;&#8230;life is Christ, and death is gain.&#8221; But I&#8217;m not in Paul&#8217;s league. There&#8217;s also my particular judgment, and I&#8217;ve wandered off-topic.<sup><a href=\"https:\/\/brendans-island.com\/catholic-citizen\/health-care-directive\/#2\">2<\/a><\/sup><\/p>\n<p>Where was I? Health care agents. Family. Autopsy. Death. Or the 20th. Or the 21st to date.<\/p>\n<p>Scything and gathering grain gave way to mechanical reapers.<\/p>\n<p>We lost an ancient tradition, but harvested more food in less time.<\/p>\n<p>I figure that helped folks sidestep famines.<\/p>\n<p>I&#8217;ve also read that famines declined because capitalism replaced feudalism. And that bigger bureaucracies led to better food distribution. Then there&#8217;s the British Agricultural Revolution and Green Revolution.<sup><a href=\"https:\/\/brendans-island.com\/catholic-citizen\/health-care-directive\/#3\">3<\/a><\/sup><\/p>\n<p>There&#8217;s probably a grain of truth in those assertions. And that&#8217;s another topic. Topics.<\/p>\n<p>As far as I know, nobody blamed the first cholera epidemic on Fulton&#8217;s steamboats or the Louisiana Purchase. Maybe because it didn&#8217;t spread beyond China and the Mediterranean.<\/p>\n<p>All of which happened shortly before 1817.<\/p>\n<p>Folks living in Bengal fingered Ola Bibi and Kali as being responsible.<\/p>\n<p>British troops got blamed for the <span class=\"\">Bundelkhand outbreak.<\/span><\/p>\n<p><span class=\"\">Seems they slaughtered and ate cattle, which offended Hindu beliefs. On top of that, the incident took place in a grove sacred to Hurdoul La.<\/span><\/p>\n<p>My view is that blaming a deity for death, disasters and disease isn&#8217;t unique to my culture. (<a href=\"https:\/\/brendans-island.com\/catholic-citizen\/sickness-death-god-love-and-questions\/#death\">February 23, 2019<\/a>; <a href=\"https:\/\/brendans-island.com\/catholic-citizen\/disasters-deaths-decisions\/#moral\">September 17, 2018<\/a>)<\/p>\n<p><a name=\"john\"><\/a><a href=\"https:\/\/brendans-island.com\/catholic-citizen\/chasing-butterflies-and-truth\/#drinking\"><img data-recalc-dims=\"1\" decoding=\"async\" src=\"https:\/\/i0.wp.com\/brendans-island.com\/blogsource\/20180105ff\/20180117-Monster_Soup_commonly_called_Thames_Water._Wellcome_V0011218-329.jpg?w=640&#038;ssl=1\" align=\"right\"><\/a>John Snow&#8217;s 1849 essay showed that tainted water carried cholera. The powers that be promptly ignored the idea. (<a href=\"https:\/\/brendans-island.com\/catholic-citizen\/power-and-climate\/#informed\">July 1, 2018<\/a>; <a href=\"https:\/\/brendans-island.com\/catholic-citizen\/using-vaccines-wisely\/#magic\">July 21, 2017<\/a>)<\/p>\n<p>By 1896, we&#8217;d had the first through fifth cholera pandemics.<\/p>\n<p>Doctors and scientists eventually learned that humorism and miasma theory didn&#8217;t match observable phenomena. That was happening during the late 19th century.<\/p>\n<p>Well-established customs die hard, so bloodletting was an accepted medical practice until the early 20th century. That may help explain why American life expectancy didn&#8217;t get much above 70 until well into the 20th century.<sup><a href=\"https:\/\/brendans-island.com\/catholic-citizen\/health-care-directive\/#4\">4<\/a><\/sup><\/p>\n<h4><a name=\"surgical\"><\/a>Surgical Successes in the Sixties<\/h4>\n<p><a href=\"https:\/\/brendans-island.com\/catholic-citizen\/disorders-decisions\/#folks\"><img data-recalc-dims=\"1\" decoding=\"async\" src=\"https:\/\/i0.wp.com\/brendans-island.com\/blogsource\/20130220ff\/20141204-800px-Indiana_Eugenics_Law_Marker-329.JPG?w=640&#038;ssl=1\" align=\"right\"><\/a>Fast-forward to the 1960s.<\/p>\n<p>The &#8216;old ways are good ways&#8217; set were having conniptions.<\/p>\n<p>No cherished belief or institution seemed safe from scrutiny.<\/p>\n<ul>\n<li>European colonies became nations<\/li>\n<li>Eugenics laws were repealed<\/li>\n<li>Miscegenation was decriminalized<\/li>\n<li>Prefrontal lobotomies were becoming unfashionable<\/li>\n<\/ul>\n<p>It was the end of civilization as we knew it. I&#8217;m not happy about every change, but on the whole I think we&#8217;re living in a (slightly) better world.<\/p>\n<p>Brouhaha over drug-resistant microbes, genetics and other bioethical issues \u2014 I&#8217;ll get back to that.<\/p>\n<p><a href=\"https:\/\/brendans-island.com\/catholic-citizen\/face-transplant-at-mayo\/#life\"><img data-recalc-dims=\"1\" decoding=\"async\" src=\"https:\/\/i0.wp.com\/brendans-island.com\/blogsource\/20160719ff\/20170221-PopSci1962Nov-p72-329.jpg?w=640&#038;ssl=1\" align=\"right\"><\/a>A 1962 accident severed a boy&#8217;s arm. Folks got him and his arm to a hospital. Surgeons tried, successfully, to reattach his arm.<\/p>\n<p>Christiaan Barnard&#8217;s 1967 heart transplant operation was successful. The patient survived 18 days.<\/p>\n<p>Organ transplants are less lethal these days, which I think is a good thing.<sup><a href=\"https:\/\/brendans-island.com\/catholic-citizen\/health-care-directive\/#5\">5<\/a><\/sup><\/p>\n<p>It&#8217;d be nice if everyone was perfectly healthy and weren&#8217;t missing any parts.<\/p>\n<p>That&#8217;s not the way it is.<\/p>\n<p>We&#8217;ve been using artificial limbs for millennia. (<a href=\"https:\/\/brendans-island.com\/catholic-citizen\/face-transplant-at-mayo\/#vishpala\">February 24, 2017<\/a>)<\/p>\n<p>Maybe that&#8217;s why I haven&#8217;t run across someone denouncing &#8220;Satanic&#8221; wooden legs.<\/p>\n<p>Human-to-human organ transplants aren&#8217;t just new. They involve trickier bioethics.<\/p>\n<p>Wanting to extend someone&#8217;s life by replacing a defective heart seems like a good idea. Problem is, whoever donates a heart won&#8217;t survive.<\/p>\n<p>Here&#8217;s where it gets tricky.<\/p>\n<h4><a name=\"bioethics\"><\/a>Bioethics<\/h4>\n<p><a href=\"https:\/\/brendans-island.com\/catholic-citizen\/face-transplant-at-mayo\/#replacement\"><img data-recalc-dims=\"1\" decoding=\"async\" src=\"https:\/\/i0.wp.com\/brendans-island.com\/blogsource\/20130220ff\/_77118919_p7500122-thymus_gland-spl-329.jpg?w=640&#038;ssl=1\" align=\"right\"><\/a>Let&#8217;s say that one of my kids needs a new heart. I know a surgeon, hospital and suppliers with flexible ethics.<sup><a href=\"https:\/\/brendans-island.com\/catholic-citizen\/health-care-directive\/#6\">6<\/a><\/sup><\/p>\n<p>They can locate and drygulch a suitable donor, extract marketable organs, and save my kid&#8217;s life.<\/p>\n<p>And everyone lives happily ever after. Except whoever got broken down for parts.<\/p>\n<p>That doesn&#8217;t mean organ transplants are basically wrong.<\/p>\n<p>They&#8217;re okay if expected benefits outweigh the risks. Donating organs after death &#8220;&#8230;is a noble and meritorious act&#8230;.&#8221; But killing someone to help another person is a bad idea. (Catechism of the Catholic Church, <a href=\"http:\/\/www.vatican.va\/archive\/ccc_css\/archive\/catechism\/p3s2c2a5.htm#2296\">2296<\/a>)<\/p>\n<p>Let&#8217;s look at another happily-hypothetical situation. What if one of my kids was in constant and unavoidable pain?<\/p>\n<p>Not so hypothetical, now that I think of it. Surgery left my son with an unpleasant mix of numbness and pain.<\/p>\n<p>Let&#8217;s up the ante, and say that he&#8217;s also crippled: not enjoying a &#8220;quality lifestyle,&#8221; as some said in my youth. Would administering a lethal dose of painkiller be compassionate?<\/p>\n<p>Maybe, from some viewpoints. But euthanasia is not a good idea. (Catechism, <a href=\"http:\/\/www.vatican.va\/archive\/ccc_css\/archive\/catechism\/p3s2c2a5.htm#2277\">2277<\/a>)<\/p>\n<p>The same would apply to self-administered euthanasia. Whatever it&#8217;s called, that&#8217;s suicide: and a bad idea. (Catechism, <a href=\"http:\/\/www.vatican.va\/archive\/ccc_css\/archive\/catechism\/p3s2c2a5.htm#2280\">2280<\/a>&#8211;<a href=\"http:\/\/www.vatican.va\/archive\/ccc_css\/archive\/catechism\/p3s2c2a5.htm#2283\">2283<\/a>)<\/p>\n<p>On the other hand, I&#8217;m not obliged to continue &#8216;over-zealous&#8217; medical treatment if I&#8217;m dying and ready to die. &#8220;Ordinary care&#8221; is another matter: and is an obligation. (Catechism, <a href=\"http:\/\/www.vatican.va\/archive\/ccc_css\/archive\/catechism\/p3s2c2a5.htm#2278\">2278<\/a>&#8211;<a href=\"http:\/\/www.vatican.va\/archive\/ccc_css\/archive\/catechism\/p3s2c2a5.htm#2279\">2279<\/a>)<\/p>\n<p>That can include painkillers, even if they&#8217;ll shorten life. The idea is that the goal is easing the patient&#8217;s pain: not killing the patient. &#8220;Palliative care is a special form of disinterested charity&#8230;.&#8221; (Catechism, <a href=\"http:\/\/www.vatican.va\/archive\/ccc_css\/archive\/catechism\/p3s2c2a5.htm#2279\">2279<\/a>)<\/p>\n<p>I&#8217;m over-simplifying the situation.<\/p>\n<hr>\n<h3><a name=\"task\"><\/a>A Task (Nearly) Done<\/h3>\n<h4><a name=\"death\"><\/a>Death and Signatures<\/h4>\n<p><a href=\"https:\/\/brendans-island.com\/catholic-citizen\/death-funerals-and-life\/#death\"><img data-recalc-dims=\"1\" decoding=\"async\" src=\"https:\/\/i0.wp.com\/brendans-island.com\/blogsource\/20180320ff\/20180405-0065r-detail-329.jpg?w=640&#038;ssl=1\" align=\"right\"><\/a>My father died about 10 years ago. My father-in-law died last year.<\/p>\n<p>I&#8217;ll die, too, sooner or later. That thought&#8217;s been inching its way toward the front of my mind, partly due to last year&#8217;s transient ischemic attack.<\/p>\n<p>Contemplating my death isn&#8217;t among my favorite pastimes. Putting it mildly.<\/p>\n<p>That&#8217;s made filling out my Health Care Directive a less-than-pleasant task.<\/p>\n<p>But now it&#8217;s done.
